Job Description:

We’re looking for professional Onsite Visitation Advocates, with home health care experience to join our team. As an Onsite Visitation Advocate, you would concentrate on visiting and advocating for individuals living as residents in long term care facilities (nursing homes). This is an excellent opportunity to use your skills in a role where you can work out in the field on a part-time basis or full-time as additional cases are acquired in your area. Your hours are your own. Schedule your visits with flexibility while receiving all the great benefits that accompany working for Ovation Health.

Many families live a great distance away from their loved ones. They currently depend on the State to notify them when care goes below the generally accepted levels of care or when abuse or neglect occurs. Frequently by the time that the family is made aware of issues, the problem has progressed to unacceptable levels. We are the eyes and ears to these families. This position provides (NON-medical) care. Ovation Health provides families with proactive third party assessment & verification services to give them the peace of mind to know that their family member is being given the best possible care that the facility can provide.

Opportunities:

Provide emotional support to patients in a manner that fosters well being.

Provide reports to Ovation Health detailing patient progress, upkeep, general well being and physical care.

Respect the autonomy and right to confidentiality of all patients and families.

Identifies the need for intervention by the facility or staff.

Documents and reports changes, (positive or negative) in patient condition to Ovation Health through reports and digital pictures.

Provides complete and accurate supporting documentation as requested.

Actively talk to, read with, visit and interact with patients in care.

Qualifications:

Prior home health experience is a plus.

The ability to effectively operate in a medical environment

A current occupation as a Home Health Aide is a plus!

Strong desire to be recognized and rewarded based upon your evaluation results.

The ability to operate within a changing fast paced environment.

College degree preferred but not required.

Verbal and written communication and skills that are clear and professional.

The ability to communicate with family members as needed.

Computer literate with the ability to E-mail reports as needed

Energetic, enthusiastic and goal driven.
