Job Description:

This is an independently contracted part-time/per diem position, perfect for the educated professional not desiring full-time employment with benefits, but seeking a gratifying autonomous position where their knowledge of the healthcare system and local geriatric services is appreciated and fully utilized. As our business grows, the Geriatric Care Manager will have a case load of client's that they will be managing through the continuum of care.

Minimum Job Requirements:

  • Registered Nurse, MSW or LCSW licensure without restrictions in AL, NY, FL or GA
  • Case/Care Management or Home Care experience
  • High level of comfort with computers
  • A passion for caring for the elderly is a must
  • The Geriatric Care Manager directs the care plan and interventions designed for specific individuals as put forth by the resident in care and their family members. This effort may include assessment, counseling, coordination of needed services, assigning and ensuring quality delivery of care-giving within residential settings, and assistance with placement if requested.

    Desirable requirements for this position are as follows:

  • Either a Bachelors’ Degree in Social Work, Gerontology, or related field or a Registered Nurse Degree with experience in Geriatric Care Management.
  • Experience in care management, home care, long-term care or other senior-service related field is preferred.
  • A working knowledge of clinical geriatric care issues, family dynamics and interventions, education and training, supervision, quality assurance and customer service is desirable.
  • Monitoring and implementing approved Plans of Care with external Care Coordination vendors and family members.
  • Knowledge of Care Coordination, working with geriatric and chronic illness
  • Knowledge of the Long Term Care industry desirable
  • The ability to work within multi-disciplined teams complicated by family interactions
  •  Excellent communication skills
  • Attending meetings and communicating with families as needed to inform and update them
  • Additional desirable assets include:

  • A background working in a long-term-care/geriatric setting
  • Case management experience
  • The ability to provide guidance to families in interpreting assessment and Plan of Care information
  • Responsibilities:

  • Promote the physical, psychological and social well-being of resident
  • Converse with resident to promote social interaction and provide emotional support.
  • Respond promptly to and positively to family requests for assistance.
  • Assist with resident functional evaluations to determine if residents’ current level of care is adequate
  • Assist the residents’ family with developing future life plans for resident.
  • Coordinate new resident orientation process and assist with resident move-ins.
  • Function as liaison to and communicate effectively and regularly with residents’ health care providers, residents and their family members.
  • Complete written reports and routine resident record reviews and updates as scheduled.
  • Demonstrate integrity, compassion and professional business practices.
  • Maintain positive and effective channels of communication.
