issues. Clients are typically referred by
other providers, by community members and
organizations, and even by self-referral. We
also accept referrals from family members,
physicians, elder law attorneys, nurses,
senior health specialists, pastoral care
departments and home healthcare agencies –
basically anyone that has a vested interest
in the health care of a senior or individual
in care or in need of care.
When you make a referral to Ovation Health you are referring to a professional care management organization. A specialist will contact you to get additional information about the client. Once we have enough basic information and have the go-ahead to move forward then an initial on-site assessment will be done. This in-depth assessment will seek to put care management solutions in place to assist the client and sometimes even their family if needed.
