Ovation Health provides comprehensive care management services. Our services are designed to meet the full range of needs presented by people experiencing healthcare issues. Clients are typically referred by other providers, by community members and organizations, and even by self-referral. We also accept referrals from family members, physicians, elder law attorneys, nurses, senior health specialists, pastoral care departments and home healthcare agencies – basically anyone that has a vested interest in the health care of a senior or individual in care or in need of care.

When you make a referral to Ovation Health you are referring to a professional care management organization. A specialist will contact you to get additional information about the client. Once we have enough basic information and have the go-ahead to move forward then an initial on-site assessment will be done. This in-depth assessment will seek to put care management solutions in place to assist the client and sometimes even their family if needed.
